Why does ferritin matter more than haemoglobin?

This is the single most useful thing to understand about iron testing.

Your body protects haemoglobin. When iron intake falls short, it does not immediately reduce your blood count. It draws on stored iron first, quietly depleting the reserve while the haemoglobin stays normal. Only once the stores are exhausted does anaemia appear.

This means a person can feel exhausted for months, be told their haemoglobin is fine, and still be genuinely iron deficient. Ferritin catches that stage. A complete blood count alone does not.

One important catch with ferritin

Ferritin rises during any infection or inflammation, regardless of iron status. So someone who is both iron deficient and unwell can show a misleadingly normal ferritin.

This is why iron studies are interpreted as a set rather than a single number. In genuine iron deficiency the pattern is characteristic: ferritin low, serum iron low, TIBC high, transferrin saturation low. Your doctor may also check CRP to see whether inflammation is distorting the picture.

Iron deficiency is a symptom, not a diagnosis. The important question is always why the iron is low.

In a young woman with heavy periods, the answer is usually obvious and treatment is straightforward. In a man of any age, or a woman past menopause, unexplained iron deficiency needs investigation, because slow blood loss from the digestive tract is a recognised cause and can be the first sign of something that needs finding early.

Taking iron tablets without asking that question can correct the blood count while leaving the underlying cause unaddressed.
