What is the HbA1c test?

HbA1c stands for glycated haemoglobin. When sugar circulates in your blood, some of it attaches to the haemoglobin inside your red blood cells. The more sugar there has been, the more haemoglobin becomes coated. Because red blood cells live for around 120 days, measuring how much of your haemoglobin is coated gives an average of your blood sugar over roughly the previous three months.

This is what makes the test so useful. A fasting glucose test is a photograph of a single moment, and that moment can be affected by what you ate yesterday, how you slept, or whether you are unwell. HbA1c is closer to a summary of the whole season.

For people already diagnosed with diabetes, the goal is different. Most adults are advised to keep HbA1c below 7 percent, though your doctor may set a higher or lower target depending on your age, how long you have had diabetes, and other health conditions. Why does HbA1c matter so much in Pakistan?

Pakistan has one of the highest rates of diabetes in the world, and a very large share of cases go undiagnosed for years. The condition is often silent in its early stages, quietly damaging blood vessels, kidneys, eyes and nerves before any symptom appears.

Because HbA1c needs no fasting and can be taken at any time, it removes the single biggest practical barrier to getting tested. For many people it is the difference between finding the problem at the prediabetes stage, when it can often be reversed, and finding it years later after complications have started.

Do you need to fast for an HbA1c test?

No. This is one of the main advantages of the test. You can eat and drink normally, and attend at whatever time suits you. Fasting is only necessary if your doctor has combined HbA1c with other tests such as a lipid profile or fasting glucose, so check your full prescription.

When can an HbA1c result be misleading?

This is important and often overlooked. Because the test measures haemoglobin, anything that changes your red blood cells can distort the result. Tell your doctor and the laboratory if any of the following apply to you:
