Quick answer: A brain CT is fast and is the right first scan in an emergency, because it detects bleeding and skull fractures within minutes. A brain MRI takes far longer but shows soft tissue in much greater detail, making it better for tumours, early stroke, seizures and unexplained neurological symptoms.

What is the difference between a brain CT and a brain MRI?
They form images in completely different ways. A CT rotates X-rays around the head and measures how much each tissue absorbs, which makes it excellent at distinguishing dense material such as bone and fresh blood. An MRI uses a strong magnetic field and radio waves to respond to water content in tissue, which is why it distinguishes subtle differences within the brain itself so well.

Why does an emergency almost always start with CT?
Because of one question that must be answered immediately: is there bleeding?
After a head injury, or in a suspected stroke, the treatment depends entirely on whether blood is leaking into or around the brain. A stroke caused by a clot is treated with medicines that thin the blood. Giving those same medicines to someone bleeding into the brain would be catastrophic. A CT answers that question in minutes and is available around the clock, which is why it comes first.
An MRI may follow once the emergency is settled, to show the detail the CT could not.
When is an MRI the better choice?
- Suspected brain tumour, where MRI shows the extent and its relationship to surrounding structures
- Early or small strokes, which MRI detects hours before they become visible on CT
- Seizures and epilepsy, where the cause is often a subtle structural change
- Multiple sclerosis and other inflammatory conditions of the brain
- Problems at the base of the brain, including the pituitary gland, brainstem and cerebellum, which CT images poorly because surrounding bone interferes
- Persistent unexplained symptoms such as numbness, weakness, vision changes, hearing loss or balance problems
- Memory problems being investigated for dementia

Does a headache need a brain scan?
Usually not. The overwhelming majority of headaches are migraine or tension type and do not require imaging at all. Scanning every headache produces anxiety and unnecessary radiation without improving outcomes.
Doctors do image when certain warning signs are present, including a headache that begins with sudden extreme intensity, one that wakes you from sleep, one accompanied by weakness, confusion, visual loss or seizure, a headache that changes character after the age of 50, or one that steadily worsens over weeks. If your doctor has decided not to scan, that is usually a considered clinical judgement rather than an oversight.
Do you need contrast dye?
It depends on the question. Contrast highlights areas with an abnormal blood supply, which is why it is standard when looking for tumours, infection or inflammation. It is generally unnecessary for straightforward trauma, suspected bleeding or routine follow up.
CT contrast is iodine based, so tell the team about kidney problems, diabetes medication, asthma or any previous reaction. MRI contrast is gadolinium based, and kidney function may be checked beforehand.
How should you prepare?
- For either scan: bring previous scans and reports. Comparison with earlier images is one of the most valuable things a radiologist has
- If contrast is planned: you may be asked to fast for around four hours
- For MRI: declare all metal, including pacemakers, implants, surgical clips, cochlear implants or metal fragments in the eye. Some make MRI unsafe
- For MRI: mention claustrophobia in advance. It is common and can be managed
- Remove jewellery, hairpins and anything metallic before either scan
